Chapter 1, Figure 2 isn't rendered in the PDF. (FoxIt Reader, Windows 7) - but there are other instances of non-rendered figures in all chapters.

While reading the book, I noticed all the external links appear to be internal Microsoft links - hence I cannot access them.

For example, on page 13 the link 'Incremental Acceptance Testing' points to https://sharepoint.partners.extranet.microsoft.com/sites/TestingGuidance/Shared%20Documents/Draft%20Content/Thumbnails/Incremental%20Acceptance%20Testing%20Thumbnail.docx

The link then prompts me for a username\password.
